We understand that sometimes concerns may arise regarding our services, and we want to assure you that your feedback is important to us. Below is our Grievance Procedure, designed to address any issues you may have promptly and respectfully.
What is a Grievance?
A grievance refers to a complaint about our policies, procedures, or services where you feel you have been treated unfairly.
Your Rights
You have the right to voice your concerns without fear of retaliation or negative impact on your treatment. You can make a verbal or written complaint to your therapist/counselor or our Clinic Director, Joann Lundberg, directly at (971) 394-3336.
When to file a Grievance
Grievances concerning specific acts or situations must be raised within 30 days of their occurrence. If there is a delay, we will explain it, and extensions may be considered at the clinic's discretion.
Steps to Take Before Filing a Grievance
Discuss: Talk to your therapist about the change you want.
Negotiate: Work with your therapist to develop a plan for change.
Define Success: Identify what success looks like for you.
Consider Consequences: Understand the consequences, whether successful or not.
Escalate if Needed: If resolution is not achieved, repeat steps 1-4 with the clinical supervisor.
File a Written Grievance: If resolution is still not reached, you may file a written grievance.
Steps to Take Before Filing a Grievance
Notify: Inform one of the listed individuals (therapist, counselor, supervisor).
Meeting: A meeting will be arranged promptly with you and other relevant parties.
Outcome Summary: Following the meeting, you will receive a written summary of the grievance outcome.
Expedited Grievances
In urgent situations where delay may harm you, you or your guardian may request an expedited review. The Clinic Director will respond in writing within 48 hours, specifying the decision on the expedited grievance.
